Do you have a MEDICAL ISSUE which prevents you from being able to serve?
Are you
- no longer a resident of Chatham County,
- not a United States citizen,
- a full-time student in class during your jury service week?
Print, sign, and provide any requested documentation at the TOP of the General Excusal Affidavit Form. You DO NOT NEED to have it notarized. Return the form and any documentation to the Jury Services office.
Are you
- 70 years of age or older,
- the primary caregiver of a child 6 years of age or younger with no alternative childcare,
- the primary caregiver of someone over the age of 6 years who cannot be left unattended,
- presently on active military duty (or your spouse is), or
- a teacher in a home study program during your jury service week?
Print and sign in front of a Notary Public the BOTTOM of the General Excusal Affidavit Form. Your section must be notarized and completed with any requested documentation. Return the notarized affidavit and documentation to the Jury Services office OR come by the Jury Services office with your ID before your week of service and we can print and notarize the affidavit for you.
